PLDT Online Application Data Privacy Notice

PLDT Online Application ("we," "us," "this site") facilitates the submission of residential and business internet application requests. We collect and process your information to help you apply and to forward your application details to the relevant service team for review and processing.

2. What We Collect

When you use this application form, we collect:

  • Your name, contact numbers, and email address(es)
  • Your complete address
  • Your selected internet plan
  • Optional information you provide, such as landmark, preferred installation date, and notes
  • Photos of your government-issued ID (front, back, and a selfie holding your ID), for identity verification
  • For business applications: company name, business registration details, and supporting business documents (DTI, SEC, Mayor's Permit, Business Permit, Proof of Billing)

3. How We Use Your Information

We use the information you provide to:

  • Process and evaluate your application
  • Verify your identity using the documents you submit
  • Contact you regarding your application, including reminders if you don't finish submitting your application
  • Forward your application details to our service team for follow-up

4. How We Store and Protect Your Data

  • All data is transmitted over an encrypted (HTTPS) connection
  • Uploaded documents (IDs, business permits, etc.) are stored in private, access-controlled cloud storage — not publicly accessible
  • Application data is stored in a secured database, accessible only to authorized personnel through a password-protected system
  • We do not sell your personal information to third parties

5. Who Can Access Your Data

Access to your submitted application and documents is limited to authorized personnel processing applications on this platform. Your information is used solely to facilitate your application and is not shared with unrelated third parties without your consent, except where required by law.

6. Data Retention

We retain your application information for as long as reasonably necessary to process your application and for a reasonable period afterward for record-keeping purposes. You may request the deletion of your information as described below.

7. Your Rights

Under the Data Privacy Act of 2012 (Republic Act No. 10173), you have the right to:

  • Request access to the personal data we hold about you
  • Request correction of inaccurate personal data
  • Request deletion of your personal data
